Fertility Billing Services for California IVF Practices

California is home to over 80 fertility clinics and some of the most complex payer landscapes in the country. EasyRCM handles billing for California REI practices — from Blue Shield and Anthem to Kaiser and Progyny — with coding accuracy and denial rates that in-house teams can rarely match.

California Fertility Insurance Landscape

California has no comprehensive state fertility insurance mandate — meaning coverage depends entirely on employer benefit design and plan type. However, the California market has a high concentration of large self-insured employers that offer Progyny or WINFertility as a voluntary fertility benefit, creating a complex dual-payer environment.

Blue Shield of California, Anthem Blue Cross CA, and Health Net are the dominant fully-insured commercial carriers. Kaiser Permanente operates as a closed-network HMO with its own internal fertility coverage guidelines that differ significantly from commercial payers. Claims submitted to the wrong Kaiser entity — or missing the Kaiser-specific auth format — are denied outright.

Medi-Cal (California Medicaid) does not cover fertility treatments. Covered California exchange plans offer limited infertility diagnosis coverage but exclude IVF treatments. Patients with exchange plans require careful benefit verification to avoid balance-billing disputes.

Top California Payer Considerations

Blue Shield of California
Fertility benefits vary widely by employer group. Large tech employers (Google, Apple, Meta) often carry Progyny carve-outs — verify before submitting to Blue Shield.
Anthem Blue Cross CA
Requires IVF authorization submitted via Availity. Modifier 22 for complex retrievals is frequently denied without operative notes attached at submission.
Kaiser Permanente
Claims must route to the correct Kaiser region (NorCal vs SoCal). Out-of-network claims are denied at zero. Fertility coverage is limited and requires internal referral from Kaiser OB/GYN.
Progyny
High concentration in Bay Area and LA tech corridor. Smart Cycle model requires correct unit reporting. Out-of-network billing to Progyny requires a separate LOA.
Health Net / Centene
Fertility benefit limited to diagnosis and IUI for most plans. IVF coverage rare without employer rider. Verify benefit level before scheduling ART cycle.
