Surgical — IUI/ICI
58321
Artificial Insemination — Intra-Cervical (ICI)
Artificial insemination by intra-cervical injection. Less common than intrauterine insemination (IUI). The sperm sample is deposited at the cervical os rather than into the uterine cavity.
Billing Notes
ICI (58321) is distinct from IUI (58322). Do not interchange them. ICI is generally considered less effective than IUI and some payers may not cover it separately. Sperm washing may still be billed separately with 58323.
Diagnosis Codes
N97.x (female infertility), N46.x (male factor), Z31.1 (encounter for AI)
Common Denial Reasons
- IUI code (58322) billed for ICI procedure
- Not covered — payer requires IUI not ICI for benefit
- Missing documentation of injection site
Payer Notes
IUI (58322) is more commonly covered than ICI. Verify that the procedure documented matches the code billed — audit exposure is high if IUI is documented but ICI is billed or vice versa.
