Culture of oocyte(s)/embryo(s), 4 or more days with oocyte identification from follicle aspiration. This code applies when embryos are cultured to blastocyst stage (Day 5 or Day 6), which is standard practice in most modern IVF labs.
Billing Notes
89251 replaces (not supplements) 89250 when extended culture is performed. Document the culture duration in the embryology report. Most labs now culture to blastocyst by default, making 89251 the standard code in those settings.
